What is Assisted Living?

Assisted living facilities are for people who need help with Activities of Daily Living (ADLs) such as bathing, dressing, laundry, housekeeping, and assistance with medications, but wishing to live as independently as possible for as long as possible. Residents in assisted living communities are not able to live home alone but do not always require constant care. The Heritage at Lake Forest is devoted to providing excellence in service enhancing their dignity, privacy and choice. We recognize that our residents are our number one priority.

At The Heritage at Lake Forest, our highly credentialed Nurses have well-established relationships with the area’s most respected providers of quality medical care, and maintain current referral list of physicians of virtually every recognized specialty. Each resident is evaluated and goals are set for their improvement. Following physician contact, our nurse will visit with the patient and family and assess his/her needs. Under the direction of the physician, and input from the patient and family, an individualized care plan is developed. The Nurse will supervise the care and maintain ongoing contact with the physician and family. Our healthcare professionals keep documentation of prescribed medications and records of medical appointments. Our private exam room is exclusively for the use of visiting physicians, podiatrists, and other health-care professionals. In addition, we offer door-to-door transportation to and from your own personal physician or dental office. And our on-site physical, occupation and speech therapists are available every day Monday-Friday.
